- Your Medicare/DVA card
- Your private Health insurance details
- Workcover paperwork/claim number – if applicable
- Details of any previous investigations and procedures
- Previous operation reports
- Your list of current medication
- X-rays and scans
- Any other information regarding your condition.

There are 5 on site car parking spaces - please note they are reverse in only. The driveway is quite steep so we advise taking the entry and exit at an angle. Alternatively, there is free on-street parking on Nerang-Broadbeach Road.
